Family Guidelines
VISITATION
We have open visitation at Jefferson Medical Associates,
L.L.C. We ask that visitors be mindful of the time of day and
the residents' personal schedule and/or tolerance. In order to
safeguard our residents we do ask that all visitors leave by 8:00
p.m. at which time the front doors are locked. If your loved one
is critical, we will accommodate your hours.
SMOKING
No smoking is permitted inside the facility. Smoking
is only allowed in designated outside areas.
PERSONAL BELONGINGS
A resident is encouraged to bring his/her personal
effects as space allows. These sentimental "homey" appointments
help to make his/her room their personal home. They are pleasant
reminders of the past that has not been set aside. We do ask that
articles or furnishings be cleared through the Activity or Social
Service Directors, the Director of Nursing or Administrator as
we must adhere to state regulation and safety codes.
LEAVE OF ABSENCE
We are most pleased when families can take a resident
off the premises for leaves of short or long duration. All leaves
must be scheduled in accordance with the resident's medical ability.
If a leave is not medically advisable, the supervisory nurse will
advise the family and enter such into the resident's medical record.
Advance notice is appreciated so the supervisory nurse can arrange
for dispensing of medications.
Holiday leaves are encouraged. We will make every
effort to accommodate your schedule. We ask, if you have a clothing
preference, that this be made known several days before the holiday
so staff can plan their schedule in a timely manner. We may ask,
that you let us know, again ahead of the holiday, when you plan
to pick up your loved one and time of return, again, we can plan
our work flow.
FOOD POLICY & ALCOHOL POLICY
Jefferson Medical Associates, L.L.C. makes every
effort to provide three well-balanced meals each day for each
resident, according to prescribed diet. Family is responsible
to always notify nursing staff prior to leaving food. We do ask
that foods be limited to snackable items such as fruits, candies,
cookies, etc. These must, for sanitary reasons, be kept in Tupperware,
Rubbermaid, or that type of container only. This way the resident
will be able to store snackable food items in a safe, sanitary
way to enjoy after you leave. We also request that all food items
be dated.
Resident's have the right to consume a reasonable
amount of alcohol beverages at the residents own expense, unless
not medically advisable as documented in the medical record.
ROOM ASSIGNMENTS
Room assignments are made in accordance with census
availability. We make every effort to see to the comparability
of each resident allowing time for adjustments. Every effort is
made to maintain each resident in their own comfortable environment.
Due to changing health status, emergencies or census change, placement
in an alternate room may become necessary. Every reasonable effort
will be made to contact the responsible party as far ahead as
possible, prior to the change. If unable to reach the responsible
party prior to a change, then every reasonable effort will be
made after the move, informing the party of the residents new
location.
PHYSICIANS CHOICE
We have qualified physician, who will come to our
facility. Those who do choose our house physician, should hospitalization
become necessary, can elect to have our house physician continue
care through the hospitalization period or the resident can go
to the hospital of his/her choice and have the former family physician
provide coverage during hospitalization. Upon discharge from the
hospital the house physician will resume coverage.
You are welcomed to invite your physician to provide
coverage while in Jefferson Medical Associates, L.L.C. He/she
must visit the resident every 30 days, and be available for calls
on a 24 hours basis. We will make every effort to warmly accommodate
him/her.
PHARMACY CHOICE
Lo-Med Pharmacy is our pharmacy provider for all
of our prescription and emergency medication needs. A family is
welcome to use the pharmacy of their choice. The pharmacy must
be able to promptly fill medications and supply PRN medications
and/or stat medications when needed, and deliver the medications
to the nursing home within 24 hours of receiving telephoned prescriptions.
CLOTHING
The residents wear their own clothing which should
be marked prior to or upon admission. Due to limited space we
ask that a reasonable number of seasonally appropriate outfits
be brought. Clothing out of season should be stored at home if
possible.
Please keep in mind that the type and style of clothing
may vary from what the resident previously wore. Physical and
medical needs and limitations may dictate these changes. Any questions
or comments are welcomed. Please direct these to the Activity/Social
Service Director, Director of Nursing or Administrator.
CONTACT
Upon admission a contact person will be assigned
for each resident. This contact person will be notified by the
facility with any issues that pertain to the resident. This contact
person will also receive notification of any Care Plan Meetings,
parties or other events being held at our facility.
FINANCIAL
The Resident financial resource is due prior to
the tenth (10th) of each month.
